Fitness Program

Get the most out of your fitness activities with Be Fit. You get to choose how you want to work out, and your $360-per-year benefit will cover the costs

Activities include the following and more:

  • Fitness class fees.
  • Gym memberships.
  • Online fitness subscriptions.
  • Weight loss subscriptions.
  • Rowing.
  • Golf.
  • Bowling.
  • Tennis.
  • Pickleball.
  • Pool exercise classes.
  • Fitness trackers.

If your fees are more than $360 a year, you pay the difference. Be Fit doesn’t cover league fees, personal equipment, fitness foods like protein bars and shakes, or Medicare-covered services like physical therapy, chiropractic care, etc.

If you forget your card, if your provider doesn’t accept this type of payment or if there are difficulties in using your card, you can submit an eligible expense for reimbursement. You must file a reimbursement request up to 90 days from the date of service on the Benefits Pro Portal or by mailing a Reimbursement Request Form found under the Tools & Support tab in your online Benefits Pro Portal. You can access the online Benefits Pro Portal through Hally. If your request is filed after 90 days, the claim will not be approved.

For transactions completed in 2024, you can file a reimbursement request on the Benefits Pro Portal or by mailing a Reimbursement Request Form found under the Tools & Support tab in your online portal.

Use the Be Fit Reimbursement Form for transactions completed prior to January 2024. After January, you can file a claim on the Flex Card Member Portal or by mailing a Reimbursement Request Form found under the Tools & Support tab in your online portal.

Get fit with a $300/year fitness benefit. You can use any plan-approved fitness center. Members can use their fitness allowance funds to pay for gym membership fees and approved services.

Vision Coverage

Get access to vision services beyond what Original Medicare covers, including a routine vision exam with an in-network provider. Plus, get a $200 allowance for eyewear. (PPO Plus has a $130 allowance).

Our plans give you a benefit of up to $120 a quarter. It allows you to purchase OTC products at participating retailers nationwide and online from a catalog with hundreds of products to choose from. (Select has a $100 benefit per quarter and Freedom has a $90 benefit per quarter).

Benefits Mastercard® Prepaid Card (Flex Card)

The Benefits Mastercard® Prepaid Card (Flex Card) is a prepaid card that is loaded with a preapproved spending limit that gives you direct, upfront access to certain benefits of your plan by allowing you to immediately pay for certain benefits. You simply swipe the card to pay for your services and your coverage will be approved if you have the funds available. Please refer to the plan materials for benefits that are covered by the Flex Card.

Acupuncture

Your plan is made with acupuncture coverage beyond what Original Medicare covers to give you more access to treatment when you need it.

  • Treatment must be for headache, neck pain or lower back pain diagnoses.
  • You don’t need prior authorization.
  • You can choose any acupuncture provider.
  • A per-visit copay may apply (varies by plan).
  • You or your provider should submit claims using the information on your ID card.
